To schedule an appointment for imaging services at Florida Hospital, simply fill out the appointment request form or call our Outpatient Scheduling Department at (407) 303-2273, between 7:30 am - 6 pm, Monday through Friday to speak with a representative. Regardless of which method you choose, just follow these three easy steps.

1. Before you start, have your physician prescription (order) and your insurance card available.

2. Complete the form below. (If you choose to call us, please be prepared to provide this information in full.) 

3. Once we have your information and requested appointment date, one of our representatives will call you to confirm your appointment and time.
